More Than One Provider. A Whole Team Behind You.
In a traditional solo-physician practice, your access to care can depend on the availability, schedule, and scope of a single provider. When your provider is away or unavailable, getting the care you need may become more difficult.
With Team-Based Care, You Have More Choice
See the right provider for your needs
See the first available family physician or nurse practitioner when you need timely care, or request a preferred physician or nurse practitioner for ongoing visits.
Access a broader care team
Connect with social workers, physiotherapists, chiropractors, psychotherapists, dietitians, and pharmacists, as well as our physicians and nurse practitioners.
Receive coordinated care
Our health care professionals work together to understand your health needs, coordinate your care, and connect you with the right services and programs.
Frequently Asked Questions
Is the "Access Clinic" the same as a "Walk in Clinic"?
The GWFHT Access Clinic is NOT a walk-in clinic. The word “Access” reflects your choice in how you receive care.
