We are a collaborative, patient-centred primary care team committed to delivering comprehensive, wholistic, and high-quality health care.
As a community focused not-for-profit, led by a volunteer board of directors, we are dedicated to fostering wellness and meeting the evolving health needs of our community.

Registration Eligibility

You have valid health coverage

You must have a valid OHIP (Ontario Health Insurance Plan) card or IFH (Interim Federal Health Program) coverage.

You do not have a primary care provider

You can register if you do not already have a family doctor or nurse practitioner.

You live in our patient priority areas

We are currently accepting patients living in North York, with priority given to M2N or M3H postal code areas.

Get Well Health Team Offers

Team Based Care

Our team includes family doctors, nurse practitioners, medical specialists, registered nurses, registered dietitians, social workers, and other interdisciplinary practitioners. We work collaboratively to provide care that is centered on you.

Family Medicine Services

We can look after all your primary health care needs such as periodic exams, vaccinations, cancer screenings, well baby visits, women’s health, chronic disease management, medication management and refills, specialist referrals, and more.

Access Clinic

We provides an ACCESS CLINIC, where you can receive timely care for both your urgent and routine primary medical needs. This Access Clinic is available with online booking, phone booking, as well as on a drop-in basis.

Multiple Languages

We currently have administrative staff and practitioners who can speak English, Mandarin, Cantonese and Vietnamese. As we continue to grow, we will offer additional languages that reflect the residents of North York community.
