What is a Family Health Team?
A Family Health Team (FHT) in Ontario is a primary health care organization that provides patients with access to a range of interdisciplinary health services, while supporting primary care providers such as family physicians and nurse practitioners in delivering accessible and team based care to their patients.
These teams provide primary care services tailored to the unique needs of the community, with a focus on:
- Health promotion and overall wellness
- Illness prevention through proactive care
- Chronic disease management, including support for mental health and addictions, diabetes education and care, and the prevention of other long-term conditions
What does Get Well Health Team Offer?
Get Well Health Team was created with team-based care and patient connection in mind. When you join our team, your primary health care is provided by a group of professionals working together to support your needs.
Our team includes family doctors, nurse practitioners, registered nurses, dietitians, social workers, physiotherapists, chiropractors, psychotherapists, pharmacists. By working collaboratively, we provide care that is centered on you—ranging from day-to-day health needs to chronic illness management and prevention programs.
As an attached patient, you’ll receive coordinated and wholistic care that is tailored to your health and wellness goals.
In addition to coordinated, holistic care tailored to your goals, we offer an Access Clinic for both urgent and routine needs—available by online booking or drop-in.
Who is my family doctor?
At Get Well Family Health Team, your care is delivered by a collaborative team of family physicians, nurse practitioners, and interdisciplinary health care providers. Together, they function as your family doctor, working as one team to support your comprehensive health needs.
As a Family Health Team patient, you benefit from care options that are not available in a traditional solo-physician model. When you need family medicine services, you have the flexibility to choose:
- The first available family physician or nurse practitioner for timely and convenient care;
- The most appropriate health professional or program to address your current health concern;
- A Family Health Team physician or nurse practitioner you prefer for ongoing visits, when available.
All members of the team have access to your shared care records, ensuring they are familiar with your health history and care plan. This team-based approach allows for seamless, coordinated care—so you are always supported.
With a dedicated team of physicians and health care providers working together, you will never be alone without family doctor care.
Is the Access Clinic a Walk in Clinic?
The GWFHT Access Clinic is NOT a walk-in clinic.
It is a Family Health Team service delivered by our family physicians and nurse practitioners, working together to provide comprehensive family medicine care. This includes preventive care, chronic condition management, and timely support for urgent medical concerns.
The word “Access” reflects your choice in how you receive care. Through the Access Clinic, you can choose to book an appointment with:
- The first available family physician or nurse practitioner for timely and convenient care; and/or
- A Family Health Team physician or NP you prefer for ongoing visits, when available.
This team-based approach ensures you receive the right care, at the right time.
I am registered and accepted as a patient, what do I do next?
Once you receive your welcome email, you are accepted as a patient with Get Well Family Health Team. This means you have immediate access to our care team and services.
Booking an Appointment
If you have urgent health concerns or ongoing (chronic) medical needs:
Book an appointment with our Access Clinic online or by phone.
You will be seen by a physician or nurse practitioner. Please bring your medical records, list of current medications, and names of specialists you see, if available.
Important Information
For patient safety and quality care:
- Narcotic medications are not prescribed
- Sleeping pills, stimulants, and psychiatric/psychoactive medications are not prescribed at the initial visit
- Forms such as ODSP social assistance and Special Diet forms are not completed at the initial visit
These requests require an established provider–patient relationship and a full review of your health history. In order to have these needs properly assessed and managed, you must first build an ongoing care relationship with the team and at least one of our primary care providers for ongoing regular care. This helps ensure safe, appropriate, and comprehensive care for you.
If You Have Chronic Conditions
If you have on-going health conditions:
A nurse practitioner or health team member may contact you by phone for an intake consultation to understand your health story. You may be invited to book a periodic physical Checkup with one of our physicians or nurse practitioners
Periodic Physical Checkup
You may choose to book a periodic physical exam through online appointment booking if you feel it would be helpful for prevention and long-term health planning.
Programs and Workshops
As a GWFHT patient, you may choose to participate in:
- Chronic condition management programs
- Preventive care services
- Health promotion workshops
These services are designed and implemented by our interprofessional health providers to support your overall health and well-being.

- Visit the FHT at either clinic locations with your medical records
- Review your medications
- Ask questions or share concerns
A member of our health care team will assist you or connect you with the right provider or program.
Your Care, Your Choice
At Get Well Family Health Team, you are in control of your health care. You can choose to access the right care, in the right place, at the right time for you. Being part of our team gives you the options to coordinate services that support your physical, emotional, psychological and overall well-being.
What do I do when I have urgent care needs?
You can book an appointment online, over the phone, or simply drop in to our Access Clinic at Get Well Clinic, 649 Sheppard Avenue West during its open hours. This helps you avoid unnecessary trips to the emergency room and long wait times.
How do I book non-urgent, routine primary care appointments?
You can book your appointments through online booking or over the phone.
What kind of primary health care services are available?
Get Well Health Team can look after all your primary health care needs such as periodic health examination, vaccinations, cancer screenings, well baby visits, women’s health, chronic disease management, medication management and refills, specialist referrals, and more.
Are all primary health care services covered by OHIP?
While OHIP covers most necessary primary health care services, there are some services that are not insured. These may include sick notes, certain forms and documents, missed appointments, letters for massage therapy, travel immunizations, copies or transfers of medical records, and more. A fee applies for uninsured services.
Our fees may change from time to time, so we encourage you to check the posted fee schedule in our centre or speak with our front desk team for the most up-to-date information.

What are the benefits of team based care vs. traditional solo-physician model?
In a traditional solo-physician practice, your health care depends on a single provider. Access to care is limited by that one person’s availability, schedule, experience, and scope of practice. If your provider is away, ill, on vacation, or retires, you may be left without care and forced to search for a new provider on your own.
With a team-based care model, you are in control of your health care.
At Get Well Family Health Team, you can choose to:
- See the first available family physician or nurse practitioner for timely and convenient care
- See a Family Health Team physician or nurse practitioner you prefer for ongoing visits,
- Access a coordinated team of health professionals and special programs, including nurses, social workers, physiotherapist, chiropractor, psychotherapists, dietitians, chiropodist and pharmacist.
All members of your care team know your health story and work together to support you. Your care is coordinated, collaborative, and centred around you.
Your health takes a village—and with a village of health care providers, you will never be without care again.
Is there free parking available?
Yes, the Get Well Health Team Centre has client parking at the back of the building. The Access Clinic located in Get Well Clinic has free parking at the back of the building. Both locations are easily accessible by public transit. There is also free street parking nearby.
What languages are available?
Our team speaks English, Mandarin, Cantonese, and Vietnamese, with more languages as we grow. For other needs, we provide a real-time online translation service covering hundreds of languages.
Get Well Health Team sounds great! I really want to register but I already have a family doctor or nurse practitioner. Can I still join?
Unfortunately, because you are already attached to a primary care provider, you are not eligible to join Get Well Health Team at this time. Our team is currently accepting patients who do not have a primary care provider, with priority given to residents in the M2N and M3H postal code areas. Please contact Health Care Connect or your local Ontario Health Teams for more options.
I don’t have a family doctor or a nurse practitioner as my primary care provider, how do I register?

Am I automatically accepted once registered?
The information you provide in the registration form helps us determine your eligibility to join Get Well Family Health Team for team-based care. We may contact you if additional details or clarification are needed. Please note that completing the registration form does not guarantee acceptance. Further instructions are provided once the form is submitted.
Do I need to bring my OHIP card?
Yes, you will be asked to show your OHIP card every time you visit Get Well Health Team or our Access Clinic. Please ensure your OHIP card is valid and up to date.
I don’t have OHIP coverage, can I register?
For uninsured patients, Get Well Clinic can provide services with applicable fees. You can also access private interdisciplinary care from Keep Well Clinic.
I am a refugee with IFHP coverage, can I register?
Refugees’ health care needs may be covered by the Interim Federal Health Program (IFHP). If you have valid IFHP coverage, please register HERE
