What is GWFHT?
At Get Well Family Health Team, we provide team-based primary care, where every patient is supported by our entire group of physicians and interdisciplinary health professionals. Unlike traditional clinics where each patient is assigned to a single doctor, our shared-care model ensures that your health is cared for by the whole Family Health Team.
How the team works
Shared Care by the Team: You have access to all the physicians and nurse practitioners on the team.
At GWFHT, you can choose to:
See the first available family physician or nurse practitioner for timely and convenient care
See a Family Health Team physician or nurse practitioner you prefer for ongoing visits.
- Interdisciplinary Collaboration: Registerd nurses, dietitians, pharmacist, social workers, chiropractor, physiotherapist, psychotherapists, chiropodist, and traditional health practitioners work together with our physicians and nurse practitioners to provide comprehensive and wholistic care.
Programs & Workshops: GWFHT offers chronic condition management programs and health promotion workshops designed to support optimal health, promote wellness, and provide preventive care.
Coordinated Care Plans: All providers have access to your shared care record, and collaborate to ensure your care is seamless, even across different time and locations.
