Registration Eligibility
You have valid health coverage
You must have a valid OHIP (Ontario Health Insurance Plan) card or IFH (Interim Federal Health Program) coverage.
You do not have a primary care provider
You can register if you do not already have a family doctor or nurse practitioner.
You live in North York
We are currently accepting patients who live in North York. Priority is currently given to individuals living in M2N and M3H areas.
