If you haven’t requested to join our Family Health Team, please complete our registration form first. 

Please note that online booking is available only to accepted patients. It can take up to five business days for us to set up your patient profile after acceptance. If you are unable to book an appointment online five business days after being accepted, please give us a call. We are happy to check your profile status. 

 

I am registered and accepted as a patient, what do I do next?

Booking an Appointment

If you have urgent health concerns or ongoing (chronic) medical needs:Book an appointment with our Access Clinic online or by phone.You will be seen by a physician or nurse practitioner. Please bring your medical records, list of current medications, and names of specialists you see, if available.

**Important Information**

For patient safety and quality care:

  • Narcotic medications are not prescribed
  • Sleeping pills, stimulants, and psychiatric/psychoactive medications are not prescribed at the initial visit
  • Forms such as ODSP social assistance and Special Diet forms are not completed at the initial visit

These requests require an established provider–patient relationship and a full review of your health history. In order to have these needs properly assessed and managed, you must first build an ongoing care relationship with the team and at least one of our primary care providers for ongoing regular care. This helps ensure safe, appropriate, and comprehensive care for you.

If You Have Chronic Conditions

If you have on-going health conditions:
A nurse practitioner or health team member may contact you by phone for an intake consultation to understand your health story. You may be invited to book a periodic physical Checkup with one of our physicians or nurse practitioners

Periodic Physical Checkup

You may choose to book a periodic physical exam through online appointment booking if you feel it would be helpful for prevention and long-term health planning.

Programs and Workshops

As a GWFHT patient, you may choose to participate in:

  • Chronic condition management programs
  • Preventive care services
  • Health promotion workshops

These services are designed and implemented by our interprofessional health providers to support your overall health and well-being.

  • Visit the FHT at either clinic locations with your medical records
  • Review your medications
  • Ask questions or share concerns

A member of our health care team will assist you or connect you with the right provider or program.
