Well Baby Program

At the Get Well Family Health Team, our Well Baby Program provides comprehensive preventative care for infants and young children from birth to 2 years of age. We are committed to partner with families to support healthy growth and development during the most important years of a child’s life. 

Our program is led by our Family Physicians and Nurse Practitioners and supported by nurses. We follow the evidence-based recommendations of the Rourke Baby record and use developmental screening tools to monitor your child’s growth and development.

What to Expect During a well Baby Visit

Our team will: 
  • Monitor your child’s growth, including weight, length/height, and head circumference
  • Assess developmental milestones and overall well-being
  • Review feeding, nutrition, sleep, and daily routines
  • Discuss parenting questions and concerns
  • Review Public Health Ontario’s routine childhood immunization schedule
  • Provide health promotion and safety education
  • Identify concerns early and connect families with appropriate resources and services when needed

Area of Focus

Growth and Development

We conduct regular assessments to track your child’s growth and confirm they’re meeting age-appropriate developmental milestones.

Immunizations

We support families in following Public Health Ontario’s publicly funded childhood immunization schedule and provide education about vaccine preventable diseases. Vaccinating your child and keeping them on schedule is one of the best ways you can protect them from serious, preventable diseases. Immunizations help guard against whooping cough (pertussis), meningitis, chickenpox (varicella), measles, mumps, rubella, and tetanus, among others.

Nutrition and Feeding

We provide guidance on breastfeeding, formula feeding, introducing solid foods, healthy nutrition, and common feeding concerns. Our team works closely with a registered dietitian, and we’ll make a referral as needed to support your child’s nutrition and feeding journey.

Health Promotion and Safety

Our topics may include safe sleep practices, car seat safety, injury prevention, oral health, physical activity and play.

Family Support

Our team is available to answer questions, provide education, and support parents and caregivers through early years of your child’s development.

Recommended Well Baby Visit Schedule

Appointments are typically scheduled as follows:
  • Newborn (within 1 week), 1- 2 weeks,
  • 1 month (optional),
  • 2 months,
  • 4 months,
  • 6 months,
  • 9 months (optional),
  • 12 months,
  • 15 months,
  • 18 months, and
  • 24 months.

Additional visits may be recommended based on your child’s needs.
