Diabetes Management Program
Our Diabetes Care Program helps patients understand, manage, and take control of their blood sugar to improve their overall health. We provide comprehensive support through a team of family doctors, nurse practitioners, dietitians, and pharmacists working together to meet your individual needs
The Diabetes Care Program includes:
- Personalized care plans for diabetes management or pre-diabetes prevention
- One-on-one follow-up with our team of interdisciplinary healthcare providers
- Support from our clinical pharmacist with medication and insulin, lifestyle changes, and help with planning and goal-setting with our dietitian.
Patients with diabetes join via a physician or nurse practitioner referrals through Access Clinic Booking. Our goal is to empower you to stay healthy, prevent complications, and achieve your personal health goals through education, understanding, assistance, and support.

Services included
Registered Dietitian (RD)
Provides a personalized nutrition plan to help you reach your blood sugar goals by planning and making dietary changes that fit your taste and lifestyle.
Clinical Pharmacist
Provides medication reviews, insulin education, and recommendations for dosage or dose adjustments in collaboration with a physician or NP. The clinical pharmacist also helps in complication prevention.

Learn and Take Control – Join Our Pre-Diabetes Workshops
We also offer free community workshops on topics like nutrition, stress management, and overall wellness. These are open to everyone, no referral needed, and a great way to start building the skills to maintain your health and prevent disease.

How does this Diabetes Care Program works?
You will start an initial consult with pharmacist and dietitian, including regular foot check and review diet,medication,insulin if applicable. Follow up every three months to review blood work result with family doctors/Nurse Practitioner and previous goal with our interdisciplinary team.
