Access Clinic (Daytime Hours)
The Get Well Family Health Team Access Clinic is open to serve you and your family’s health care needs.
You can book an appointment online or call us at 416-977-1560.
The GWFHT Access Clinic ensures you experience convenient and quick care. Our team provides continuous and comprehensive care for you and your family to keep you in optimum wellness and your health on track.
Access Clinic (Evening/Weekend Hours)
Our extended hours care service is available by appointment only on weekday evenings and Saturday mornings.
- Appointment Required: Please call 416-977-1560 or book an appointment online
- Available: Monday tto Thursday – 5 p.m. to 7 p.m., Saturday mornings – 9 a.m. to 12 p.m.
Physical Check up
A physical check-up provides a comprehensive review of your physical health including routine screenings, preventative assessments, and overall wellness evaluation.
Appointments are required for physical check-ups.

Is the "Access Clinic" the same as a "Walk in Clinic"?
The GWFHT Access Clinic is NOT a walk-in clinic. The word “Access” reflects your choice in how you receive care. You can choose to book an appointment with the first available family physician or nurse practitioner for timely and convenient care. Or you can follow up with a specific provide for ongoing visits, when available.

At Get Well Family Health Team, your care is delivered by a collaborative team of family physicians, nurse practitioners, and interdisciplinary health care providers.
Together, they function as your family doctor, working as one team to support your comprehensive health needs.
As a Family Health Team patient, you benefit from care options that are not available in a traditional solo-physician model. When you need family medicine services, you have the flexibility to choose:
the first available family physician or nurse practitioner for timely and convenient care, the most appropriate health professional or program to address your current health concern, a Family Health Team physician or nurse practitioner you prefer for ongoing visits.
All members of the team have access to your shared care records, ensuring they are familiar with your health history and care plan. This team-based approach allows for seamless, coordinated care—so you are always supported.
