• If you require a periodic health examination, please book your appointment online or by calling our office.
• If you need to see a family physician or nurse practitioner for a medical concern, please book an appointment with our Access Clinic by phone or online.

If you know someone without a primary care provider, please encourage them to complete our registration form to see if they may be eligible to join the Get Well Family Health Team.
