Thank you for completing our intake form. At this time, our Family Health Team can only accept patients who live within our catchment area so we can provide coordinated local care.

• Contact Your local Ontario Health Team  – connect with services close to home

• Contact the Walk-In Clinic at Get Well Clinic or another walk in clinic near you.

Our Family Health Team is part of Ontario’s Health Team (OHT) model, which ensures care is coordinated locally with hospitals, specialists, and community services. Being connected to a provider in your own region helps make care easier to access, well-coordinated, and fair for all patients in the community.
