Registered Psychotherapist/Counselling Social Worker

Job description

Job Type: Full time/Part Time. Supervision may be available.

Duties and Responsibilities 

  • Assess biopsychosocial needs and provide short-term, evidence-based counseling and therapy to individuals, families, and groups.
  • Collaborate with patients, primary care providers, and other FHT integrated health providers to develop and implement care plans. 
  • Develop and implement psychoeducational and skill training groups and programs.
  • Facilitate access to appropriate community and health services, coordinating referrals and helping patients navigate the broader health and social service systems. 
  • Act as an advocate for patients and families, helping to remove barriers to timely access to services. 
  • Support health promotion efforts and develop strategies to address the social determinants of health. 
  • Work as an integral part of an interdisciplinary team, collaborating with physicians, nurse practitioners, and other providers to optimize patient care. 
  • Provide intake assessment to help navigate patients to receive appropriate care within the family health team. 
  • Liaise with healthcare providers and facilitate referrals within the Family Health Team and to secondary/tertiary care services.
  • Provide written reports, detailing client’s recovery and outlining future treatment goals
  • Share assessment results and intervention plans with referring practitioners and other team members. 
  • Participate in team meetings to contribute to program development and patient care strategies. 
  • Participate in administrative work during the development stage of the family health team. 
  • Contact client for treatment follow ups.
  • Focus on patient-centered care, ensuring the client’s well-being is prioritized. 
  • Engage in self-reflective practice to adjust their approach and meet the diverse needs of patients. 
  • Take on additional initiatives that contribute to the growth of the health team.

Education and Qualifications

  • Masters Degree in Counselling Psychology or Social Work
  • Current registration with the College of Social Workers and or College of Registered Psychotherapists
  • Minimum of two years of clinical/community based experience
  • Excellent people skills
  • Highly developed communication skills, both written and oral
  • Self-motivated
  • Trained in therapeutic modality such as CBT, DBT, and EFT.
  • Ability to work collaboratively with a multi-disciplinary team
  • Can work independently in a flexible environment
  • Must demonstrate strong assessment and treatment skills within Anti-Oppressive and Recovery Practice and include expertise in one or more of: Cognitive Behaviour Therapy, Interpersonal Therapy, and Mindfulness-based Therapy, Dialectical Behaviour Therapy, and Emotion Focused Therapy. Must be willing to engage in relevant additional training, as required to meet the needs of the program. 
  • Experience and/or competency to provide services for individuals from indigenous, diverse cultural backgrounds and 2SLGBTQIA+ communities. 
  • Working knowledge of the Personal Health Information and Privacy Act, Health Care Consent Act, Mental Health Act, Substitute Decision-Makers’ Act, Child and Family Service’s Act, Access for Ontarians with Disabilities Act, Regulated Health Professionals Act and community resources is required 
  • Expertise working with people experiencing symptoms of low mood and anxiety 
  • Experience in supporting individuals who have experienced trauma and skills/experience in the delivery of Trauma Focused Therapy would be considered an asset 
  • Experience in addictions counselling is an asset 
  • Evidence of ongoing professional development and continuing education 
  • Second language skills such as Mandarin, Cantonese, Korean, Farsi, Tagalog are an asset.
  • Flexibility in working hours maybe required, with some evenings and weekends to meet patient needs 
  • Completion of a Vulnerable Person Sector reference check
