Registered Dietitian
Job description
We are seeking a Registered Dietitian (RD) to join our Family Health Team. The RD will provide individualized nutrition counselling, support chronic disease management, and contribute to health promotion initiatives. This role involves both one-on-one patient care and group education, in collaboration with the broader care team.

Duties and Responsibilities
- Collaborate with patients to assess nutritional needs, considering dietary habits, medical history, cultural, socio-economic, and psychosocial factors.
- Conduct nutrition and anthropometric assessments to guide care planning.
- Develop, implement, and evaluate individualized nutrition care plans tailored to patient needs and context.
- Provide evidence-based nutrition counseling, diabetes education, obesity and weight management, and co-create self-management plans with patients.
- Work closely with the multidisciplinary team to deliver holistic, patient-centered care.
- Support Chronic Disease Prevention and Management programs through outreach, education, and counseling.
- Uphold professional standards, ethics, and organizational values as set by the College of Dietitians of Ontario.
- Engage in lifelong learning, reflective practice, and continuous quality improvement to enhance patient outcomes.
- Maintain program records, including diabetes program data and quarterly reporting.
- Contribute to cross-functional initiatives and perform other duties as required.
- Take on additional initiatives that contribute to the growth of the health team.
Education and Qualifications
- Bachelor of Science in Nutrition or equivalent.
- Completion of a Dietitians of Canada–accredited internship/practicum program or equivalent, preferably with a community focus.
- Registered and in good standing with the College of Dietitians of Ontario; eligibility for membership with the College of Dietitians of Ontario and Dietitians of Canada.
- Certified Diabetes Educator (CDE) designation preferred.
- Minimum three years of relevant experience, or at least two years as a Registered Dietitian in a community-based health organization.
- Strong understanding of the social, economic, environmental, and cultural factors influencing nutrition and health.
- Excellent interpersonal, verbal, and written communication skills.
- Strong organizational, time management, and problem-solving abilities.
- Demonstrated experience in health promotion program planning and implementation.
- Familiarity with client-centered and self-management approaches to chronic disease prevention and management is an asset.
- Current CPR/First Aid certification.
- Fluent in English, additional languages reflecting the community (e.g., Cantonese, Mandarin, Farsi, Korean and Tagalog) are an asset.
- Ability to work flexible hours, including some evenings and weekends.
- Current vulnerable sector check (within one year).
