Registered Nurse(RN)

Job description

Job Type: Full time or Part time. Work hours can include weekdays and rotational weekends.

Duties and Responsibilities

  • Perform nursing assessments and interventions within RN scope of practice.
  • Administer medications, injections, immunizations, and wound care.
  • Support chronic disease management programs (e.g., diabetes, hypertension).
  • Provide health education and counseling to patients and families.
  • Collaborate with the interprofessional team to optimize patient care.
  • Provide clinical intake, triaging, and system navigation services to patients.
  • Liaise with healthcare providers and facilitate referrals within the Family Health Team and to secondary/tertiary care.
  • Ensure practice aligns with professional standards, ethical guidelines, and organizational policies.
  • Engage in continuing education and reflective practice to enhance patient care quality.
  • Perform cross-functional duties and other assigned tasks as needed.
  • Promote positive public relations and support a family-centered care environment.
  • Assist with clinic administration, including clinical coaching, inventory management, and maintaining supplies, medications, vaccines, and PPE.
  • Documentation for medical, statistical, and administrative purposes.
  • Maintain accurate and confidential patient records and documentation.
  • Participate in group programs, quality improvement initiatives, and team meetings.
  • Take on additional initiatives that contribute to the growth of the health team. 

Education and Qualifications

  • Bachelor of Science in Nursing  from an accredited institution.
  • Registered and in good standing with the College of Nurses of Ontario.
  • Valid CPR HCP certification.
  • Minimum 2 years’ experience in community or primary care, including chronic disease management and health teaching.
  • Strong interpersonal, organizational, and multitasking skills; detail-oriented and team-focused.
  • Excellent telephone and customer service skills.
  • Proficient in Microsoft Office; EMR experience (e.g., Oscar EMR) is an asset.
  • Flexible availability, including some evenings and Saturdays.
  • Able to work at multiple locations within North York
  • Fluent in English, additional languages reflecting the community (e.g., Cantonese, Mandarin, Farsi, Korean and Tagalog) are an asset.
  • Valid vulnerable sector police record check completed within the past year.
