We are sorry that you are not eligible as you are already connected to care.
Thank you for your interest in registering with the Get Well Family Health Team.
At this time, we are giving priority to people who live within the Greater Toronto Area (GTA)and do not have a family doctor or nurse practitioner as their primary care provider (PCP).
If things change and you no longer have a primary care provider such as your family doctor is retired or you completed the de-rostering process with you family doctor, you are welcome to resubmit a request to join our family health team. We’d be glad to support you.
What can you do next?
- If you already have a PCP, please continue your care with them. Your PCP knows your health history, has your records, and can best coordinate your care.If your primary care provider is unavailable and you need urgent medical attention, you may visit Get Well Clinic’s Walk-In Clinic or another walk-in clinic near you.
- If you are thinking about switching to another PCP or family health team, we encourage you to speak with your current provider first to see if your concerns or challenges can be addressed together.
