Eligibility to Join Our Team
Eligibility Criteria
- You have a valid Ontario Health Insurance Plan (OHIP) card or Interim Federal Health Program (IFHP) coverage
- Live within the Greater Toronto Area (GTA) and be able to attend in-person appointments at our clinic.
- You do not have a family doctor or nurse practitioner as your primary care provider
Patients living in North York will be given priority. If you live in another part of North York, you are still welcome to submit a request form, and your application will be considered based on availability.
How to Register
- Please register by filling out our online registration form.
- Please have your OHIP card or IFH document ready.
Need help Registering?
If you have trouble completing the online registration form, you are welcome to visit our clinic in person during office hours. A member of our team will be happy to assist you with the registration process.
