The approach is evidence-informed, practical, and patient-centred. Treatment is always tailored to each patient’s goals, lifestyle, and current abilities.

Get Well Family Health Team Physiotherapy service is grounded in a patient-centred, evidence-informed approach. Care focuses on understanding the whole person rather than treating symptoms in isolation. Education, active participation, and realistic goal-setting are emphasized to support sustainable recovery and reduce the risk of recurring injury.

Eligibility

  • Registered with Get Well Family Health Team
  • Between 19–65 years old
  • Not covered under ODSP,WSIB or through Motor Vehicle Collision (MVC) claims
  • Not covered by extended health insurance 
  • Must not be participating immediately after an overnight hospital stay due to injury

  • How do I see the physiotherapist at Get Well Family Health Team?

    Ask doctor or nurse practitioner to refer you to the physiotherapist by booking an appointment with Access Clinic. If you are not yet Get Well Family Health Team patient, please register here.

  • What should I expect at my first physiotherapy appointment?

    Your physiotherapist will ask about your concern, review your health history, and do a physical examination. Together, you'll set goals and make a plan for your remaining sessions.

  • What if I'm not eligible for the service?

    There are several free options available to Ontario residents can help connect you with a physiotherapist:

    • OHIP-Funded Community Physiotherapy - If you're not eligible for physiotherapy with the family health team, you may qualify for free physiotherapy at a designated community clinic.     • Home-Based Physiotherapy - If getting to a clinic is difficult, physiotherapy can be delivered in your home or retirement residence. Contact Ontario Health at Home to find out if you're eligible via phone at 310-2222.
    • Private physiotherapy with insurance coverage - If you have extended health benefits through work or school, you can typically access physiotherapy through a private clinic with or without a doctor or nurse practitioner referral.
    • Work-related injuries - These are covered through the Workplace Safety and Insurance Board (WSIB) when you injure yourself at work.
    • Motor vehicle accidents (MVA) - Your treatment is funded through your auto insurance provider when you are involved in a motor vehicle accidents.
  • Why is Family Health Team physiotherapy only available to a certain population?

    Get Well Family Health Team physiotherapy is part of a publicly funded healthcare program in Ontario, which means it’s designed to support people who may not have access to other forms of coverage. Because funding is limited, eligibility criteria are set to make sure services go to those who need them most. The goal is to avoid duplication of funding and ensure that public healthcare resources are used fairly and efficiently.
